Requirements

Company must be based in an EU, EEA, EFTA, or DCFTA member country or in the UK — this includes any parent/holding company
Hosting providers may not simply resell/sub-host on infrastructure of a non-EU/EFTA company (e.g. configuring servers on AWS does not qualify)
Open-source projects that can be self-hosted are also listed; the geography criteria do not apply to them but the product must be reasonably easy to self-host
Suggestions are only accepted through the site's suggestion tool after registering an account — the site explicitly says product/category suggestions sent via chat or email will not be processed

Evidence the company (and any parent company) is based in EU/EEA/EFTA/DCFTA/UK
Which existing category the product fits — new categories cannot currently be suggested
Product name, URL, and description for the suggestion tool

The optional 'EU hosted' label has its own stricter criteria (core services hosted in the EU by EU-based companies; US-owned CDNs/DDoS providers on the critical path break eligibility)
